Dr. Habib does not participate with any insurance plans. Payment is expected at the time of service. An itemized bill with procedure and diagnosis codes will be provided at each visit. This can be submitted to insurance companies for reimbursement purposes.

 

Missed Appointments

There will be a $25 fee for appointments cancelled with less than 24 hours notice.

 

Prescription Refills

Please allow 3-5 days notice.  We endeavor to fill all prescription requests rapidly but this may not always be possible.  Please have your pharmacy fax refill requests to 410-823-8381.

 

Please note that all patients on chronic prescription medications need to have a complete physical exam annually.
